2025 Troop Reservation Form Use this form to make an initial reservation. Do not use it for making any other payments.
2025 Troop Payment Form Use a new copy of this form each time you make a payment (other than your initial reservation deposit). Payment is by Troop or Crew only, not for individuals.
Provisional Scout Application If a Scout cannot come to camp with his own troop, we will place them with a host Marin Council unit during any session week chosen.
BSA Annual Health & Medical Form A/B/C(this 2019 edition is the most current and the only one accepted). A completed 3-part form must be submitted by all Scouts and adults spending any time overnight at Camp Marin-Sierra. (Those visiting only during the day must bring at least Parts A & B for check-in). 2025 Special Dietary Needs Request FormMust be emailed or faxed to the Marin Council office at least two (2) weeks prior to your arrival in camp; please bring an additional copy with health forms.
2025 Patrols Count for Jamboree Meals FormBring this form with you and present at camp check-in to let us know how many people are in each patrol of your unit for jamboree cooking (every Wednesday or all-week).
2024 Activity Release Form This specific Camp Marin-Sierra form is required for every Scout under 18 years old who wishes to engage in the restricted activities listed on the form. No other form will be accepted at any time.

From Interstate 80, take Exit 161 onto State Hwy 20 passing the Cal-Trans maintenance station; CMS is the second driveway on the right.
From Grass Valley/Nevada City heading East on State Hwy 20, it is one of the last driveways on the left before you arrive at the on-ramp to Interstate 80.
